Prepare for Your First Visit

Your first consultation is an opportunity to explain your symptoms, review your medical history, and discuss your goals with Dr. Deena Kuruvilla. Preparing a few details beforehand helps us make the most of your visit.

  • Bring a photo ID, a current medication and supplement list, allergies, and relevant medical records.
  • Include prior neurological evaluations, imaging reports, laboratory results, and treatments you have tried. Ask the office how to transfer records securely.
  • Write down your main questions and the changes you hope to achieve.
  • If headaches are a concern, note how often they occur, how long they last, associated symptoms, and which treatments help.
  • Confirm the appointment time, location, visit format, and fees with our office.

Keep a Simple Symptom Diary

A diary can help you describe patterns more accurately. Record the date, duration, symptoms, effect on daily activities, and any medication you used. You can also note sleep, meals, menstrual timing, stress, or other factors you think may be relevant.

Bring your notes to the appointment. A diary supports your evaluation; it does not diagnose a condition or establish that a particular factor caused your symptoms. Do not change prescribed treatment based on a diary without discussing it with your clinician.
