This policy explains how information is handled through brainhealthct.com, the website of Brain Health Institute in Westport, Connecticut. It applies to website visits, general inquiries, appointment requests, and newsletter signup requests.

This website policy is separate from the practice’s Notice of Privacy Practices for patient health information. Contact our office to request that notice or ask about your medical-record privacy rights.

When you use a contact form, request an appointment, or write to us, you may provide your name, email address, telephone number, scheduling preferences, and the information you choose to include in your message. A newsletter signup request provides an email address. Please provide only the information needed for your inquiry.

Do not submit detailed health histories, medical-record attachments, Social Security numbers, payment-card details, or other sensitive information through general website forms or ordinary email. Ask the office for a secure method to send clinical information.

Submitting a general inquiry does not enroll you in a newsletter. To stop newsletter communications, use an unsubscribe option when provided or contact our office.

For medical-record access, amendments, or other patient privacy requests, ask the office for the appropriate process. Please do not email identification documents or clinical records unless the office has provided a secure method.

Children and Caregiver Inquiries

Parents and guardians should contact the office on behalf of children rather than have children submit personal information through website forms. If you believe a child has submitted personal information without appropriate permission, contact us so the request can be reviewed.
